This issue has been plaguing our board for years. I just finished dealing with it and searched the query hoping for a fix.
It is random, we can go months, or hours after the fix. Once it happens connections to the database server soar and they never complete. They fill up all the connection pool to the database and the site goes down. We have 2,335,962 users on our board.
